The new partnership offers Conventus insured physician members access to Navvis’ integrated population health management capabilities with one of the largest New Jersey commercial health plans. These services are provided by Navvis’ Value-Based Services Organization, which is committed to achieving a new national standard in value-based care performance. Recently, the organization announced top-tier results in Florida, scoring among the top 6% nationwide of 541 participating ACOs.
Physicians will gain access to analytics, technology, and multi-disciplinary care coordination services, as well as support to optimize their practices. This exclusive complimentary offer to Conventus primary care physician practices helps them have access to the infrastructure and support needed to successfully participate in commercial and Medicare Advantage value-based care programs through Per Member Per Month (PMPM) payments and potential shared savings. Conventus will provide hands-on assistance in coordinating and streamlining workflow processes across other value-based care programs, such as MIPS and other commercial carrier value based programs, to avoid duplication of work.
