Esse Health Strengthens Partnership with Navvis as its Value-Based Care Partner to Further Advance and Accelerate Performance
St. Louis, Missouri, December 2, 2025 – Esse Health, one of the country’s highest performing physician enterprises, today announced an enhanced partnership with Navvis, the leading value-based care enablement company serving health systems, physician enterprises, and health plans nationwide. Building on a strong foundation as part of the Surround Care family of companies, Esse Health and Navvis are deepening their partnership, with Navvis selected as Esse Health’s value-based care partner.
The expanded partnership will accelerate Esse Health’s already outstanding performance in value-based care. The expanded partnership allows Esse Health to further leverage Navvis’ value-based care enablement expertise and operational acumen. More specifically, Navvis will support Esse Health in the following ways:
- Actionable insights, unifying analytics across disparate platforms to deliver actionable insights, creating a seamless approach to measurement and performance;
- Care coordination, enhancing referral practices and post-acute care management to improve quality and efficiency for patients especially during transitions of care;
- Education and training, elevating on-boarding and continuing education to equip clinical and operational teams with tools for long-term success;
- Operational excellence, aligning reporting, planning, and physician practice operations to ensure consistent system-wide performance; and,
- Leadership and culture, building on Esse Health’s strong governance and culture of excellence to drive alignment, accountability, and continuous improvement.
Esse Health and Navvis have also partnered on a key initiative, the Surround Care Academy, a comprehensive physician training program for health systems, physician organizations, and health plans to significantly improve physician and system-wide performance in value-based care.
